Mentavi Health offers a formal and comprehensive assessment. All assessments are evaluated by a psychologist licensed in the state you reside in. To provide continuous improvement in assessment comprehensiveness, quality, and diagnostic capabilities, our assessment is just one out-of-pocket cost of $189.
This cost includes taking our formal assessment, receiving a potential diagnosis of ADHD, depression or anxiety and a personalized report written and signed by your diagnosing psychologist. An itemized statement or "superbill" is also included within all of our assessment results so that patients have the opportunity to seek reimbursement for the cost through their insurance carrier. Please contact your insurance carrier before taking our assessment to see if your plan reimburses for mental health, tele-health services.

Initial Appointment: $199
After receiving results from our completed assessment you and your clinician will discuss options and develop your personalized Medical Treatment plan together.
Check in on Progress: $175
30 days after initial appointment. You and your clinician will discuss your Medical Treatment plan progress and identify any necessary changes.
Schedule Follow-Ups: $175
Every 90 days for a routine follow up. Appointments are scheduled every 90 days for safety and best practice.
Note:
Providers may require an earlier appointment for medication or dosing changes or any change in your medical condition.
Receiving three prescriptions within this 90 day period is not guaranteed and depends on various factors.
Utilizing our 3rd party partner, Klarna, is one way to pay for the cost of our assessment and treatment services. Klarna allows individuals to break the entire cost of the service down into 4 monthly payments instead of paying the full cost up front. Klarna is an available payment option to all patients at check out!
Klarna does run a soft-check on your credit, but this will have no affect on your credit score! Once you are approved for your 4 interest-free payments, you can purchase your service now and pay later. We do not participate nor directly bill insurance companies. However, you may be able to get reimbursed from your health insurance company, possibly up to 100% of what they typically cover. To find out if you can get reimbursed, please call your insurance company. You can also send them an itemized receipt showing what services were provided and what you paid for them.
If you have an HSA or FSA account for your health insurance, our services are qualified expenses, so you can use that money to pay for them.

We do not accept Care Credit health and wellness cards.
Helpful Tip: Occasionally our card reader may not recognize the HSA/FSA card and payment may be declined. There is no need to be alarmed! Please contact one of our patient advocate support specialist by Live Chat on our website and they will be more than happy to assist you with the payment process.
Mentavi Health does not directly participate with any insurance companies, but some insurance plans may reimburse for "out-of-network" services. Please contact your insurance provider to check coverage and get reimbursement information.
We provide an itemized statement, also known as an itemized receipt or superbill, that contains the necessary billing codes needed to file a claim.
Itemized Receipts for Assessments:
The itemized receipt for your completed assessment is located towards the bottom of your downloaded report. You can click
here to be taken to the patient portal login.
Itemized Receipts for Appointments:
• For appointments after December 2, 2024, an itemized statement can be downloaded directly from the patient portal
and then select (Appointments > Past Appointments > See Details > Download Statement).
•For appointments before December 2, 2024, you can request an itemized receipt by submitting a form through our website or your patient portal. From the dropdown, you can select "Documents and Forms" and choose "Itemized Receipts" as the document you would like to receive. Please also include each date of service that you need on the form.
Helpful Tip:
• The initial payment receipt received via email does not have the necessary codes for insurance claims. Be sure to request or download an itemized receipt to avoid claim rejection.
• If you use a FSA or HSA to pay for services, the original payment receipts may be needed as supporting documentation along with the itemized receipt for services.
We do not bill any insurance companies directly. If you are a member of a Medicaid plan please check with your plan's coverage details to see if they will reimburse you for our assessment or any medical visits for the treatment and follow up if needed. Not all of our providers are Medicaid certified, which may influence your ability to have services with us reimbursed, or have your prescriptions paid for by your Medicaid plan.
If you choose to pay out of pocket for our assessment and diagnosis, you are able to take your results to another provider, like your primary care provider, who is Medicaid certified, and in your network. Then, if medication is prescribed, the medications would qualify for Medicaid coverage if available in your plan.
If you choose to pay out of pocket and see our providers for the treatment of your ADHD or related conditions we cannot guarantee that the medications would be covered by your Medicaid plan.
Helpful Tip: Many of our patients have had good experiences working with RxOutreach. RxOutreach is a mail order pharmacy whose mission is to provide access to affordable generic medications. RxOutreach is a cash pay system and they do not bill insurance companies. If you are interested in learning more about them, you can review their website at rxoutreach.org. Coverage depends on your insurance plan. Not all insurance providers recognize health coaching as a billable service, but many offer wellness benefits through employers or insurance plans that may cover it. Wellness coaching may also qualify for FSA/HSA reimbursement. Some providers may require a letter of necessity for reimbursement. If you need assistance with this paperwork, please contact us.
