What is the latest update from the DEA about telemedicine and controlled substances?Although the Public Health Emergency (PHE) ended, the DEA has extended the telemedicine flexibilities through December 31st, 2025. Patients can continue being treated virtually without an in-person visit just like during the PHE.
Though not mandatory, we highly recommend establishing care with a primary care provider and having at least an annual physical examination every year.
If you decide to have an in-person visit, your clinician can complete a referral form and easily send it to us via email [email protected] or fax to 616-210-3118. Alternatively, your practitioner can send a referral directly to us.
An added bonus is that this proactive step can significantly expedite the prescription filling process at some pharmacies.
How do Klarna flexible payments work?Utilizing our 3rd party partner, Klarna, is one way to pay for the cost of our assessment and treatment services. Klarna allows individuals to break the entire cost of the service down into 4 monthly payments instead of paying the full cost up front. Klarna is an available payment option to all patients at check out!
Klarna does run a soft-check on your credit, but this will have no affect on your credit score! Once you are approved for your 4 interest-free payments, you can purchase your service now and pay later. How do I request an accommodation for school or work?Living with ADHD can impact many aspects of your life, and we’re happy to assist with accommodation requests. We recommend first checking with your school or workplace to see if they have specific forms or requirements for accommodation requests.
How to Submit an Accommodation Request:
From Our Website:
- Select “Patients” from the menu bar.
- Click “Submit a Request” from the dropdown.
- Choose “Documentation Request”.
From your Patient Portal:
- Go to the Support section.
- Click the “Documentation Request” link under the “For Fastest Response” section.
Helpful Tips:
- Complete all required fields in the form.
- Attach any forms provided by your school or employer, ensuring the patient information sections are filled out before uploading.
- Include all details required by the requesting institution.
- Once your request is submitted, it will be routed to a team specialist and your clinician for processing. Please allow up to 14 business days for completion, and submit your request well in advance of any due dates.
Important Notes:
- Clinicians can only support accommodation requests for the conditions they are treating. These requests are reviewed and completed at the clinician’s discretion on a case-by-case basis.
- If your form requires a face-to-face visit, you will need to schedule and pay for a follow-up virtual visit with your clinician to complete the request.
ADHD can be diagnosed by any qualified licensed medical practitioner, including your own family doctor or pediatrician, but most patients are referred to a psychologist for a thorough assessment. Your family doctor will often require proof of diagnosis from a psychologist before prescribing medications. Our assessment allows you to be evaluated by a psychologist faster, easier, and more affordably than traditional means. In order to start treatment in our medication management program, you will need to start by taking our assessment, we do not accept previous diagnoses of ADHD at this time.
Why not just use one of the free tests online?Free screening tools are short quizzes meant to help patients understand if they need to seek further evaluation or satisfy their curiosity about ADHD. Our online assessment is reviewed by a licensed clinical doctor of psychology who is able to diagnose ADHD, as well as other associated conditions such as anxiety and depression.
How do I receive an itemized statement or “superbill” for my completed service?Mentavi Health does not directly participate with any insurance companies, but some insurance plans may reimburse for “out-of-network” services. Please contact your insurance provider to check coverage and get reimbursement information.
We provide an itemized statement, also known as an itemized receipt or superbill, that contains the necessary billing codes needed to file a claim.
Itemized Receipts for Assessments:
The itemized receipt for your completed assessment is located towards the bottom of your downloaded report. You can click here to be taken to the patient portal login.
Itemized Receipts for Appointments:
- For appointments after December 2, 2024, an itemized statement can be downloaded directly from the patient portal and then select (Appointments > Past Appointments > See Details > Download Statement).
- For appointments before December 2, 2024, you can request an itemized receipt by submitting a form through our website or your patient portal. From the dropdown, you can select “Documents and Forms” and choose “Itemized Receipts” as the document you would like to receive. Please also include each date of service that you need on the form.
Helpful Tip: The initial payment receipt received via email does not have the necessary codes for insurance claims. Be sure to request or download an itemized receipt to avoid claim rejection. If you use a FSA or HSA to pay for services, the original payment receipts may be needed as supporting documentation along with the itemized receipt for services.
How long will it take to get my results?You should usually get your results in up to 5 business days. Please keep in mind that it may take longer, depending on the state where you live and the psychologist’s workload.
What can I expect from the results of my assessment?Rest assured, your assessment is carefully reviewed by a licensed clinical psychologist, not by artificial intelligence. Our psychologists personally evaluate the information you provide to develop a diagnostic impression and apply diagnosis codes in accordance with the DSM-5-TR criteria.
While conditions like ADHD, anxiety, and mood disorders are commonly identified, our assessments go beyond diagnosis. Using additional screening tools, our psychologists craft individualized recommendations tailored to your unique needs.
This personalized process involves a detailed review of your mental health history, consideration of contributing risk factors, and careful evaluation of your personal explanations and experiences. By combining professional expertise with a holistic approach, our licensed clinical psychologists ensure you receive accurate insights and recommendations designed just for you.
Can I bring my assessment and formal diagnosis to my local provider?Yes! Many patients choose to send their assessment report to their local provider for treatment. Patients can download their completed assessment from a laptop or desktop computer to share with their provider, or the report can be faxed directly from your patient portal. The assessment report will have more than enough information to assist your provider in developing a treatment plan with you.
Helpful Tip: Psychologists can’t prescribe medications. You must form a treatment plan with a provider who is licensed to prescribe medications in your state such as a primary care physician, psychiatrist, physician’s assistant, or nurse practitioner.
If you choose to take your assessment results to your primary care provider, you will need to be an established patient with them. For example, urgent care physicians typically do not treat or prescribe medications for conditions like ADHD because they require ongoing follow-up visits for the patient’s safety.
