Requesting an itemized receipt is easy—just submit a form through our website or patient portal. In the “Documents and Forms” section, select “itemized statement” and include the service dates you need.
If you have an HSA or FSA, our services qualify as eligible expenses, so you can use those funds to pay for them.
For Medicaid or Medicare members, please see our separate FAQ on coverage. We do not accept Care Credit health cards.
