To request a refill, you must be a current medical management patient with Mentavi Health.
You can submit a refill request or scan the QR code below, which will take you to the submission form on our website. Select “medication refill request” and provide the information as requested.
Please allow 2 business days for your request to be processed. Once you submit it, you will receive an email confirmation.
