ADT Psychiatry has partnered with Mentavi Health to provide patients with access to streamlined, comprehensive, and convenient assessment and diagnostic services online for ADHD, Anxiety, Depression, and related mental health conditions. Our online Assessment gives patients answers and suggested next steps in days instead of waiting months to see a provider in person.
What to expect next
Provide us with your name and email address below and click “Submit” to create a portal account.
PLEASE NOTE: The email address you use to create your portal account cannot have been used previously with ADHD Online.
As you enter your personal information, the Portal will ask you to submit payment information. You will notice a box labeled “Promo Code”.
Enter promo code adtpsychiatry100 in this box to bypass the payment screen.
Once you complete the online assessment, include fax number 469-487-7885 to have a copy of your report sent directly to ADT.
Your Assessment will be reviewed and analyzed by a Licensed Psychologist within 5 days. You will receive an email notifying you when your Assessment results with diagnostic information and next steps are available in your portal account.
Once you receive your ADHD assessment results, it is important to take the next steps in your treatment plan by following up with the ADT Psychiatry team at adtpsychiatry.com.
